How claims are paid

Two ways your bill gets settled

Which one applies depends on the clinic or hospital and your insurer. Ask before treatment when you can.

Option 1

Direct billing

The provider sends the bill straight to your insurer.

  1. Show your insurance ID card when you arrive.
  2. The provider contacts your insurer and bills them.
  3. You may still be asked to sign a claim form or pay your deductible.
Option 2

Pay and claim

You pay the provider, then ask your insurer to pay you back.

  1. Pay and keep itemized bills and receipts.
  2. Fill in your plan's claim form.
  3. Send the form and documents to your insurer for reimbursement.

Either way, call your insurer's assistance line as early as you can. Some plans require it before treatment that is not an emergency.

Tracking your claim

Avoid a denial

Five mistakes, five fixes

Most denials are avoidable. Flip each card to see how to stay clear of it.

Mistake01

No proof of the incident

Without hospital records, police reports or airline confirmations, the insurer cannot verify what happened.

Mistake02

Missing medical documents

Claims need doctor’s notes, itemized bills and proof of payment to confirm the treatment.

Mistake03

The expense is not covered

Every plan has limits and exclusions. Costs outside them are refused.

Mistake04

A pre-existing condition

Plans without pre-existing cover can refuse claims linked to an earlier condition.

Mistake05

Filed too late

Policies set a reporting time limit. Late claims can be refused even when covered.

Explanation of benefits

Read your EOB like a pro

After your claim is processed, your insurer sends an explanation of benefits (EOB). It is not a bill. It shows how the insurer handled each charge.

  • What the provider billed
  • What the plan covered and paid
  • Any deductible or amount you owe
  • The reason for any charge that was not paid

If something looks wrong, compare it with your policy wording and call the claims number for your plan.

EOBSample layout
  • Amount billed
  • Plan paid
  • Deductible
  • Not covered
Every EOB lists a reason code next to any charge that was reduced or not paid.

How to appeal a denied claim

You can ask your insurer to look again.

  1. Read the EOBFind the exact reason the claim, or part of it, was denied.
  2. Check your policyCompare the reason with your policy wording and benefits.
  3. Gather supportAdd missing records, receipts or a letter from your doctor.
  4. Send your appealSubmit it to your insurer in writing, then reply to any questions.
